In these cases, the pain may have initially been triggered by tissue damage (nociceptive pain) or nerve injury (neuropathic pain), but becomes sustained by changes in the central nervous system (nociplastic pain).

As a result, heightened nerve sensitivity and the brain’s ongoing protective predictions can reinforce one another, maintaining or even amplifying pain even in the absence of ongoing damage.

The complexity of chronic pain can make it challenging for both individuals and professionals to clearly identify which mechanisms are driving or sustaining the pain experience, as they often overlap or co-exist, blurring the lines between cause and effect.



Nociplastic pain is the key mechanism underpinning diagnoses such as Chronic Primary Pain conditions and can also contribute to some cases of Chronic Secondary Pain conditions.

    While giving pain a name helps us make sense of what we’re experiencing and provides a way to communicate the problem, lasting relief often comes from truly understanding the person behind the pain and the deeper issues involved.

    Hope Through Understanding Pain as an Alert to Harm - Not Just Injury

    Thanks to recent breakthroughs in pain neuroscience, pain is now understand as a complex neurobiological process arising from dynamic interactions between the brain and body. Rather than simply receiving signals, the brain actively integrates factors such as past experiences, expectations, emotions, and the surrounding environment before interpreting those signals as potential harm, ultimately shaping the pain experience.

    The brain is constantly working to keep us safe, and much of this happens automatically, outside of our conscious awareness. Through a process known as predictive processing, the brain anticipates potential harm and in doing so, it can activate the same neural pathways involved in actual injury, triggering real protective responses like pain, muscle tension, inflammation, or increased nerve sensitivity, even when no physical damage is present.

    While this system serves a protective function by responding in advance to reduce the risk of harm, it can blur the line between cause and effect. This often creates a persistent feedback loop in which the brain’s predictions and the body’s protective responses reinforce one another, strengthening neural pathways and sustaining, or even amplifying, pain, sensitivity, or tension in the absence of ongoing tissue or nerve damage.

    It is this emerging understanding that suggests when pain lasts beyond expected healing times, isn’t explained by another diagnosis, or continues despite treatment, it often involves altered central nervous system processing (nociplastic pain). In such cases, the brain and nervous system may have adapted in a way that maintains protective responses through predictive processing. Essentially, the brain continues to anticipate harm, even when there’s no ongoing tissue or nerve damage.

    These recent breakthroughs in pain science have helped people better understand that pain isn’t simply something happening in the body, nor is it “all in the head.” Rather, pain is a real and meaningful protective alert generated by the brain, designed to be felt in the body and shaped by a complex interplay of biological, psychological, and social factors.

    All these vital insights highlight that pain is not only a uniquely individual experience, but, crucially, in cases of chronic pain, they underscore the urgent need for comprehensive, personalised care that takes all contributing factors into account.

    Achieving this, means looking beyond symptoms and diagnostic labels to consider the whole person, fully understanding and thoughtfully addressing the underlying messages within the brain’s protective processing that may be driving or sustaining the pain experience.

    University-led Research

    An initial feasibility study of OldPain2Go®, conducted by Teesside University, Newcastle University, and the Norton Physiotherapy Centre, has been successfully completed and published in Pain and Rehabilitation, the journal of the Physiotherapy Pain Association.

    Following the promising outcomes of this initial research, which concluded that further investigation into the efficacy of this intervention is warranted, a randomised, double-blind controlled trial is now underway, further evaluating the efficacy and safety of the OldPain2Go® approach within a clinical setting.

    Our larger-scale observational study, currently pre-published and under peer review, involved 207 individuals experiencing a wide range of long-term pain types and locations, including fibromyalgia, arthritis, chronic back pain, and more, with an average pain duration of nearly 12 years, reflecting the real-world complexity of chronic pain.

    Sessions were delivered by one of 108 trained OldPain2Go® practitioners across a variety of real-world settings, and the results were striking.

    Outcomes:

    76% reported a complete release of pain following a single session. (158 individuals)

    The remaining 24% (49 individuals) experienced an average reduction in pain of 78%.

    Hi, I'm Steven Blake, Creator of OldPain2Go®, and this is my personal message to you.

    "What began as my personal journey to overcome five decades of chronic pain has grown into something far bigger, a practical, compassionate approach now helping thousands of people worldwide break free from long-term pain.

    Diagnosed with spinal arthritis and ankylosing spondylitis at 38, I lived in constant discomfort for much of my life. But in 2011, while retraining as a therapist, I encountered a technique that had potential but also significant flaws. I adapted it for myself, and to my amazement, the pain I’d carried for 50 years disappeared. That moment changed everything.

    I couldn’t stop there. I needed to understand why it worked, and how to help others do the same. So I refined the process using logic, practice, and lived experience. Years later, the science began catching up and confirming what I had already found in practice: that pain, especially long-term pain, is not always a signal of ongoing damage."

    OldPain2Go® was born out of this understanding.

    Early Days:

    Initially I found it difficult to explain what seemed incredulous, so promoting my work wasn’t easy. I encountered natural scepticism, sometimes even hostility, which was difficult and upsetting. But I was driven by the potential to change lives, so I kept going.

    In 2016, I started running free pain clinics to refine my skills and test my evolving ideas. Later that year, I began teaching others, sharing my knowledge to empower more people.

    Since then, over 1,800 individuals worldwide have been trained, including doctors, nurses, physiotherapists, psychologists, neurologists, pain science researchers, and a diverse range of wellbeing professionals.

    In addition to training practitioners live online, we’ve delivered in-person training across the globe — from major cities to smaller communities. This includes the USA (Las Vegas), Australia (Sydney, Brisbane, Melbourne, Adelaide, Perth), New Zealand (Auckland), Belgium (Aarschot), the Netherlands (Deurne), and a wide range of locations across the UK, such as London, Ruskington, Lincoln, Cardiff, York, Chesterfield, Hull, Kegworth, Northallerton, and Swindon.
