We are excited to present our upgraded Client Feedback form!
Redesigned to be more user-friendly and convenient for remote sessions, it is now available on this public page, larger in size and streamlined to include only relevant information.
This form is a valuable tool to measure your impact and encourage clients to share their experience through video or written testimony.

Instructions for Accessing and Using the PDF Form:
In-person client sessions:
Simply click the link, download and print it out for your client to fill out manually.
Online remote client sessions:
Either forward the link above or send a downloaded copy of the form to the client. They can fill it our using the digital text boxes in the PDF (see example on the right), save it, email it back, or they can complete it manually and send you a photo of the completed form.

The form includes a message encouraging clients to help others by sharing their experiences, with a clear consent section and an option to choose their preferred method of testimony: video or written.
Video Testimonials
Video testimonials often make a more significant impact than written ones due to their ability to convey emotion and authenticity, creating a stronger connection and trust. The visual and auditory elements of video can capture tone, facial expressions, and body language, which written words alone struggle to convey.
Guide:
- Record the video in landscape mode to ensure optimal viewing quality.
- Begin by confirming that you have your client’s consent to record, as per the wording on the consent form.
- Introduce yourself and then your client.
- Ask your client to describe their pain levels upon arrival, at their worst, and at the end of the session. Encourage them to share their transformative ‘OldPain2Go® Pain Reset Moment.
Written Testimonials
Written testimonials, especially those coupled with a picture of the client, allow readers to absorb the information at their own pace and revisit it whenever needed.
Guide:
- In-person - clients can utilise the box on the feedback form to write their testimony.
- Online - Forward the PDF to your client for them to add digital text boxes and return to you.
- Encourage your client to share a profile picture to accompany their testimony to enhance the authenticity and impact
How to Showcase Your Success Stories
As awareness of OldPain2Go® grows globally, the demand for practitioners, who effectively demonstrate their impact, will increase.
Submitted testimonies can be showcased as Success Stories in OldPain2Go® publications, complete with the practitioner’s picture and link to contact details, reaching a wider global audience of people seeking an active practitioner.
